I had an all one panel glass door remade as two panes - and had white aluminium put into the bottom pane - into which the cat flap was installed. As previously said this avoids the problems with cutting the glass - which would be nigh on impossible for a double glazed door anyway. Using the white aluminium matches a modern double glazed doorframe and avoids the painting/maintenance problem.

That would depend on the type of glass door you have ?, if it is a full length single piece of glass, then you would have to get a hole cut into it which would not be easy, and whoever was going to do it would probably not guarantee they would not break it.
If it is a split glass door ie two separate panels, then replace the lower panel with marine plywood, then cut the hole.
