on the prefectures website for example for St Brieuc http://www.cotes-darmor.pref.gouv.fr/Demarches-administratives/Permis-de-conduire/Visite-medicale-du-permis-de-conduire#N530 then download the PDF list all the info is here

The Prefecture where you submit your application will give you a list together with the additional forms that you and the doctor have to complete.

You can probably ask your 'normal' doctor - the receptionist should have a list.

The test I did consisted of blood pressure taken and reading a card across the surgery (eye test). That was it, and cost €35.
