Sorry, can't help with whether or not you need the mutuelle in place first, but we changed insurance company about six months after our first daughter started her treatment with no problem whatsoever. The second insurance company just carried on with the payments, no question at all.

As far as the level of cover goes - get the maximum you possibly can - and make sure it is fully explained to you, and clarify any questions - we changed our mutuelle because we were told we had 100% cover for orthondontie, which in fact was not the case at all!  We now have 200% cover on our mutuelle so are pretty much fully reimbursed for all expenses.
