Puppy innoculations and micro chip done at 10 weeks and 2nd injection at 12 weeks with rabies vaccine can travel 21 days later after vet has wormed and health checked puppy not later than 48hrs before travel

Be sure your vet puts the time of worming and health check on the passport as if it is an hour or two over time you will not be able to travel

I had this problem a few months ago when returning to UK as my vet had put the date but not the time So please double check

I recently had my dog done in anticipation of a trip to the UK. 

The puppy injections require a booster vaccine after one month, the rabies vacine can be done at the same time on the first visit as long as the microchip is put in place first.

after rabies vaccination it is possible for your dog to travel after four weeks. So yes  no problem in traveling. 

Once you have it done a visit is required to your bets between days 1-5 before travel to provide worming treatment and vet to say animal is in good health to travel.
