Sorry to be flippant.

I'm not sure what you mean by the Assurance Maladie renewal documents. Assurance maladie simply means health insurance and health insurance comes from many different sources, each of which would issue their own documentation. So without knowing what health insurance scheme you are in, we have no idea what documents you're talking about. Are you covered by the state scheme, or privately? If you're in the state scheme then ameli.fr might have an English page on their website, I'm not sure. If you're in a private scheme it will depend on the insurer, there are some companies that specialise in expats so no doubt they could send a translation of the documents but I doubt that all assurance companies would have their documents translated into English. However, if you have a private mutuelle then renewal normally happens automatically, it's cancelling it that people usually find difficult!

The figure is not the issue.  Assurance maladie requires a renewal which I have never done, this is my first time however I am told that it is an annual requirement.  My question to be more specific is the following: "is there an English translation anywhere of the renewal documents".

Unfortunately the figure at the bottom of your avis would be the same in any language...
