If you mean is obtaining an EHIC card the only way to ensure that he can get medical treatment abroad under the NHS, well yes AFAIK it is. He needs to prove to the provider that the NHS will cover his treatment, and the EHIC card is that proof. If he hasn't applied for an EHIC, he isn't covered by the NHS for treatment abroad.. Getting an EHIC really couldn't be simpler.

If you mean, is online the only way to get one, no it isn't - the phone number to ring is on the website Rabs gave, and they are very helpful when you ring up.

The EHIC card is free. However there are some scam websites that charge you for making an application and don't send you a card, so be careful not to use one of those.

Your friend might also want to take out travel insurance as the EHIC covers medical treatment but not e.g. the costs of repatriation.

He can apply for a European Heath Insurance Card on line which costs about £17. It takes about a week to come. The website is :-   www.nhs.uk/ehic
